The Graffiti Kids After School program is a low-cost but high-quality ministry for children in Pre-K 4 to 5th Grade that provides Christ-centered after school care Mondays-Thursdays from school dismissal to 5:30 pm. After school begins September 16, 2019.
We currently only pick up at the Earth School, PS 64, The Children's Workshop School, and East Village Community School. If your child attends a different school, then please contact us before continuing your application.
If your child has an IEP, 504 Plan, or has special needs, then please apply in person to determine eligibility.
For new or returning children: if your family receives Public Benefits, then you are eligible to enroll in our scholarship program. Please apply in person at Graffiti Church during the hours of 11am to 5pm, Monday through Thursday, with proof of guardianship, Public Benefits card, and a current budget letter stating that you are currently receiving benefits.
We allow up to 6 adults (over 18 years old) to be listed as authorized to pick up your child/children from our After School Program. Please indicate below if you need to add persons, and continue to do so for up to 6 people.
We are excited your child will be joining us for the Graffiti After School Program. You will be required to sign a Release of Liability, which will be provided for you in person, in order to complete enrollment. We have also included our Cancellation Policy and Online Agreement Consent. If you have any questions or concerns, please contact us before finishing your application.
I certify this child does not require an IEP, 504 Plan, and does not have special needs or require special care. If your child has an IEP, 504 Plan, or has special needs, please apply in person to determine eligibility.
I understand if I am late picking up my child from Graffiti Kids After School, then I am required to pay an additional $5.00 for every 5 minutes after 5:30 pm. The additional payment must be paid within 3 program days of the late pickup date in order for my child to continue attending the after school program.
I understand that acceptable behavior is a condition of my child’s enrollment in Graffiti Kids After School. I understand my child must follow the rules, policies, and procedures outlined in the Parent Orientation Packet. Unacceptable behavior will be corrected verbally, with time-outs, with meetings with parent/guardian, and lastly through suspension/expulsion from Graffiti Kids After School with no refund eligibility.
I will attend a 30-minute parent orientation. I will choose to attend either Tuesday, September 24 at 5:30 pm or Wednesday, September 25 at 11:00 am. I understand that if I do not attend, then I am still responsible for reading and abiding by the policies outlined in the Parent Handbook that will be given to me. I will be required to sign and return a form indicating my compliance.
